How Can I Become An AFC Member?

howcaniAn AFC Member must:

Live in Massachusetts and be receiving MassHealth Standard or CommonHealth Insurance, or have Senior Care Options through an Accountable Care Organization (ACO) that is contracted with Beacon Adult Foster Care, Inc., such as Commonwealth Care Alliance (CCA) or Tufts Health Plan.

An AFC Member must require:

Daily cueing and supervision and/or assistance with at least one Activity of Daily Living (ADL) – such as bathing, dressing, ambulating, transferring, eating, or toileting – due to a medical, physical, cognitive, or mental condition.

An AFC Member may also require:

Daily management of one or more behaviors – such as wandering or socially-innappropriate, disruptive, or abusive behaviors.
